Joint Accreditation (JA) Standards for Integrity and Independence in Accredited Continuing Education require accredited providers to collect, review, and document financial relationships with ineligible companies for all individuals who can control or influence educational content, including planners, faculty, presenters, reviewers, and moderators. This form is used to disclose relevant financial relationships from the previous 24 months, identify potential conflicts of interest, and support the review and mitigation process to ensure accredited education remains independent, evidence-based, balanced, and free from commercial bias.
